As a nurse or case manager, who are you there to serve? people who suffer from severe mental diseases that frequently coexist with issues in other spheres of their lives. These clients are becoming more capable of preserving their position in society as a result of therapies that employ the FACT approach.
As a nurse or case manager, you are aware with the primary duties and responsibilities. At GGz Centraal, a case manager's responsibilities include:
. use the FACT technique to provide clients with outpatient support.
. offer helpful assistance in the areas of employment, relationships, and money.
. Engage the client's network and family in the course of treatment.
. Create a rehabilitation package while letting clients choose their own objectives.
. You may manage your own time and are in charge of completing your own agenda (working from home is a possibility).
Your group:
The interdisciplinary FACT team North is forward to meet you and your new coworkers. We provide outpatient care and support to those with serious mental health issues alongside you. The client receives assistance and treatment in their own surroundings.

Your new location of employment? The Kattenbroek neighborhood of Amersfoort is home to FACT team North. From here, we provide services to the municipalities of Eemdijk and Bunschoten-Spakenburg as well as the northern districts of Amersfoort.

. a gross monthly income based on 36 hours that ranges from €2,857 to €4,434 (CAO GGZ, job category 50).
. a 24-hour-per-week, permanent contract. Since our business is built on trust, you will immediately be given a permanent contract.
. an 8.33% end-of-year bonus and an 8% holiday allowance.
. 35 life-phase hours and 166 leave hours are calculated using a 36-hour workweek.

What you carry with you is this:
You are autonomous and able to keep a clear outlook. With a shared caseload, you will treat clients separately at home for the majority of your working hours. Additionally, you have excellent communication abilities, which help you keep up positive connections with network partners, clients, and coworkers. For this position, you also have:
. a nursing degree earned via higher professional education (HBO). We encourage you to apply if you are registered as a mental health professional and have finished social work, social work practice, or social work.
. a related BIG registration
. familiarity with (outpatient) mental health services.
. a clear understanding of and connection to working with EPA clients.
